Benefits of UP Family Benefit Scheme

A compensation of Rs 30,000 will be provided by the state government to families living below the poverty line
All the benefits of the death assistance scheme will be directly to such families under the above circumstances

The age of the head of the family should be between 18 to 60 years

National Family Benefit Scheme Guidelines

You need to fill your form legibly in English
He or she needs to provide details of a national bank account
Cooperative bank account won’t be valid to apply for National Family Scheme
The income certificate issued from Tehsil level will only be valid
All the information needs to be correctly filled by the applicant
Applicant will have to upload the copy of their documents at the time of filling their application form
Only the death certificate issued from recognized hospital will be valid
The signature on photo of the beneficiary should be no more than 20kb and in the JPEG format
Documents like death certificate of the deceased, bank passbook, identity card of the beneficiary should not exceed 20 KV
